How to Request My Medical Records
To request medical records, you may visit Health Information Management (HIM/Medical Records) on the main hospital campus, Support Services building. A Release of Information Clerk will be available to process your request between the hours of 8:00 AM and 4:30 PM, Monday through Friday. Feel free to call a Release of Information Clerk at 225-924-8216 or 225-924-8127 for questions.
To request access your records, download, complete and return Woman’s Hospital Patient Request for Access to Health information form [pdf]. Return this completed form with a copy of your valid driver’s license or valid picture id by fax, mail, or in person. Communications via email over the internet are not secure. Health Information Management handles medical record requests for itemized bills, Radiology/Imaging records, Laboratory, Maternal-Fetal Medicine, Woman’s Center for Wellness, and all other Woman’s outpatient clinics.
Birth Certificates
For birth certificate information, including status of your baby’s birth certificate application, please contact us at 225-924-8128 or 225-924-8753. For information regarding obtaining a birth certificate, you may contact the Louisiana Bureau of Vital Records and Statistics at 1-504-593-5100. You may call the Social Security Administration office at 1-800-772-1213 or 1-866-613-3070. You may also visit the Social Security Administration website for additional information.
Physician Practices
Doctors are in independent practices from Woman's Hospital. Contact information for doctors with privileges at Woman’s is listed in our Doctor Directory, searchable by name, group, and specialty. We do NOT handle medical record requests for physician office practices. Each physician office handles its own requests for medical record copies. Call the doctor’s office to check if the doctor’s practice offers an electronic medical record service.
