At the conclusion of this offering, the participant will be able to:

  1. Identify the componenets of an integrated care model.
  2. Describe key components for a successful integrated care model implementation, as well as barriers to successful implementation.
  3. Cite examples of tools used to screen patients and identify their referral needs (medical, community resource, etc.)
